This pro-forma reply slip is a letter of response addressed to a charitable company limited by guarantee (CCLBG) from a member of the CCLBG either confirming or correcting the information that the CCLBG has in relation to a member’s full name and service address.

This reply slip should be sent by the CCLBG to each member at the same time as the letter explaining the new requirements of the Economic Crime & Corporate Transparency Act for full names of members to be included on CCLBG registers, including the duty on members to notify this information to CCLBGs.
